Integrated care and patient empowerment are central pillars of the action plan agreed as part of the EIP pilot on active and healthy ageing. This workshop shed light on these topics through interventions by the EC and members of the eHealth Innovation Thematic Network. Examples of successful patient coaching and integrated care were presented highlighted. The plenum discussion has identified barriers and enablers for further deployment of integrated care services. The second part of the workshop was focused on prevention and co-production of health, i.e., not limited to ill people, and not necessarily involving conventional healthcare services.
